Case history

James, a 62-year-old male, was brought to the emergency department due to weakness of his left upper extremity, left facial paralysis and slurred speech. His daughter says he was in his usual healthy state when suddenly he was unable to lift his arm while he was his eating dinner.

James was diagnosed with hyperlipidemia 16 years ago and hypertension 12 years ago. He has been put on rosuvastatin, lisinopril, and aspirin as his maintenance medications but he admits to not being compliant. James is a truck driver and states that he does not exercise and watches TV all day during his spare time. His diet consists of fast food or Chinese take- away since his wife died 4 years ago.

Physical examination and laboratory tests

His temperature is 37.4 deg C, blood pressure is 150/100, heart rate is 93/min, and respiratory rate is 17/min. His BMI is 32.

Neurological examination shows loss of sensation on the left side of the face with decreased strength on his left upper extremity. CT scan reveals occlusion of the right middle cerebral artery indicating Ischemic stroke.
