The proposal should include the use of a QI tool (e.g. Fishbone Diagram)to support the submission, and the student will include this in an appendix.
The student is not required to implement a project.

Reference this section to show that you understand the evidence base (research)
What research evidence is there to justify the change being proposed and why might the change lead to improvement??
What might be the impact of making an improvement?

Who will your stakeholders be and what is their role in your QIP?
What are the anticipated benefits of involving these stakeholders (for them and for the project itself)?
How many patients, service users, and members of the public or staff would you need to involve to know if the project was working and over what timeframe?
What specific QI tool or method will you utilise?
Include the Tool used as an appendix (i.e., fishbone/driver diagram/ process map)
Critically reflect on the use of a fishbone diagram, driver diagram, logic model or process map and explain how you will use it/them
What measures will determine if the change(s) made during your proposal would make an improvement?
Measures or evaluation methods should be listed and explained
This section is where you tell us how you will measure the change you are making. Think about how you will collect data (it may be qualitative, quantitative or a combination as appropriate for your proposal), who it will be collected from and at what point(s) you will do this. You must describe what type of measures they are and reference your discussion appropriately.

What is your plan for leading and managing change throughout the QI process? You should draw from the leadership and change management literature in this
Explain your plan for a series of small incremental changes. Refer to the PDSA cycles described in Canvas and lectures.
Discuss one or more PSDA cycles as appropriate

Have you considered the cost of implementing your change for the organisation, can this be sustained?
What are the proposed strategies to ensure that any positive benefits of the change(s) are maintained longer term?
Plans for sharing learning from your project (Dissemination)
Why is it important to disseminate learning?
Who within and outside of the organisation might be interested to find out how your QIP
