Purpose :

The purpose of this assignment is to provide students with the opportunity to demonstrate knowledge in client safety through the application of effective interpersonal and professional communication techniques with the client and health care team. Students will identify communication barriers and client risk factors and analyze how patient-safe communication strategies may be implemented to improve client outcomes.

Scholarly Written Paper

1. Identify a situation where you believed your client’s safety was at risk because of improper communication during any of your clinical placement in the practical nursing program. Use evidenced based literature to support your rationale that the situation was unsafe.

2. Implementing a Patient-Safe Communication approach, identify one (1) barrier and the two (2) client related risk factors that affected the communication in the incident. Explain how the barrier and risk factors affected the communication in the situation that was identified. Use evidenced informed literature to support your understanding of the one (1) barrier and two (2) risk factors

3. Discuss three (3) Communication Strategies that could be implemented for the situation to create positive outcomes. Selected strategies must be strongly linked solutions for the identified problems in the situation. Evidenced based literature must be used to support the rationale for each of the identified strategies
